The sterilization cycle parameters recorded in production differed from what was specified in the validation report. The in-process inspection points prescribed in the quality plan were not being performed. This gap is the source of most regulatory non-conformities. The fix is straightforward: ask for a batch record from a recent production run and compare it against the technical file before you commit to a purchase order.
